340B: Failing to Serve the Communities Who Need It Most
November 2, 2026
Distraught woman worried about 340B Drug Pricing Program

The 340B program was billed as a program intended to help underserved communities with a simple promise: give drug price discounts to hospitals and clinics who serve our most vulnerable, and in turn, those hospitals will use the savings to provide care to those who can least afford it. Unfortunately, in the past 30 years, that promise has gone unkept. And it is minoritized communities who are paying the price.

Local and national communities are speaking out, urging federal and state policymakers to implement much-needed reforms to stop the abusive practices and better align the 340B program with its original intent.

“Inadvertent loopholes have been utilized, increasing profit margins for the hospital system while they have reduced services in one of our predominantly Black communities. It is immoral to benefit off the backs of Black and Brown residents under the guise of healthcare, and it must cease immediately.” - Richmond Mayor Levar Stoney

Reforming the 340B program will be a tremendous step forward for health equity and accountability, so that communities no longer have to tolerate the abusive practices that have enriched large hospitals and corporations to the tune of billions of dollars.

“... hospitals and pharmacies are free to exploit the program for profit while leaving patients—especially those in lower-income and marginalized communities— without access to the medications they need. Health equity demands that 340B savings be reinvested into patient care rather than corporate profits.” - Clayola Brown

By ensuring that 340B revenues stay in the communities they were always intended to serve, our neighbors will lead healthier lives and reverse the years of consolidation and corporate takeovers that have resulted in hospital closures in our neighborhoods. Working families - nurses, caregivers, and laborers who depend on these very hospitals and clinics deserve a program that actually delivers on its promise, not one that pads corporate bottom lines on their backs.

Congress and state legislatures must listen to these and other community voices who are demanding change.
